Camp Registration is accessed and completed online using the link above.

There are a few  additional required documents. These may be downloaded and printed from the home page of your online account, or you may do so below.   

All are due by MAY 15, 2026. 

Once completed and signed, UPLOAD directly to your online account (PREFERRED METHOD), or send the completed documents to the camp by mail, email or fax.

  1. Physician's Form - Use this form or send us a copy of your camper's most recent physical (within one year of camp)
  2. Medications Orders Form - Use this form or send us a copy of a SIGNED ORDER from physician for all medications to be administered during camp, Medications Orders MUST BE SIGNED BY PHYSICIAN.
  3. Immunizations Record - please send/upload a current copy of your camper's immunization record
  4. Multiple Permissions Form - Use this required form for: Permission to participate/acknowledgment of risk, photo release, swimming, sunscreen and insect repellent permissions
  5. NYS Children's Camp Brochure - from the NYS Department of Health.  INFORMATIONAL ONLY
